Care Manager

The ideal candidate is an empathetic individual interested in a mix of management duties of the house operations, staff and volunteers as well as direct care to ensure a comfortable and caring residence for terminally ill people in their final days. Prior experience in health care and experience managing and scheduling staff or volunteers is required. Outstanding communication and organizational skills needed. Proficiency and comfort with Microsoft Suite required. Must be able to travel locally. Ability to be on-call required.

The mission of Gateway House of Peace is to provide a safe, comfortable and caring residence for terminally ill people while supporting their families during their final days. For the community members who become our residents, this is often the only option for their final days. Gateway House of Peace provides this care at no charge to the resident or their family.

A diverse and rewarding role with the chance to truly make a difference. Duties include oversight of house operations including resident care (management and direct), referrals, staff and volunteer development, scheduling and supervision.

The Care Manager will:

  • Responsible for managing staffing coverage including weekends and holidays.

  • Interview potential residents and their families and provide recommendation to the Executive Director to placement at Gateway House.

  • Schedule New resident admissions

  • Interact with Hospice to facilitate resident’s care.

  • Continually communicate with staff and family members as required about the resident’s health and keep accurate records of communications.

  • Maintain Rx counts and review/ensure medication logs are properly completed.

  • Interview for staff member positions and provide hiring recommendations to the Executive Director.

  • Screen potential volunteer’s w/Volunteer Coordinator.

  • Coordinate student programs with Volunteer Coordinator.

  • Responsible for covering “call-offs”.

  • Coordinate and provide training to new staff, annual trainings for current staff and volunteers. Maintain all training documents.

  • Conduct periodic staff meetings.

  • Provide Executive Director with staff concerns, any disciplinary actions that may be required and perform annual evaluations of staff.

Care Manager Requirements:

· 2+ years managing staff

· 1+ year Patient Hands on Care experience

· LPN’s, RN’s encouraged to apply

· PT hours, flexible depending on experience/availability.

· Salary: $20.00 - $22.50 per hour

· Excellent organizational skills

· Outstanding communication skills, both written and verbal

· Ability to travel locally
